Observation reports should detail the following: Identifying client information:

Information about who provided the supervision,

The date, time, and duration of contact,

Who attended the visitation,

An account of critical incidents,

A summary of activities by the parent and child,

Comments or requests made by the parents or child,

Interventions made during the visit.
